An informal drop-in session for LD & Send families run by an NHS Social Prescriber to help find the right support and services for you.
An LD and SEND Children and Families Social Prescriber is someone who helps children and young people
with Learning Disabilities (LD) and Special Educational Needs and Disabilities (SEND) — along with their
families — to find the right support and services.
We work closely with families to understand their needs and connect them with local resources, activities,
and services that improve health, wellbeing, and everyday life.
We can support children who have a diagnosis or who are on the pathway to get a diagnosis.
